Referral and Appointment 

Please read before submit the referral

1. Referrals (Getting Started)

  • You must have a referral from a GP or relevant specialist to be considered.

  • The referral must:

    • Be addressed to this clinic/practitioner

    • Match the services they provide

    • Be legible and in PDF format

  • We do NOT accept:

    • Referrals to other doctors/clinics

    • Non-relevant referrals

    • Illegible or non-PDF files

    • Referrals from nurse practitioners

  • Important:

    • Referrals are only accepted via online form (not email or fax) on website.

    • Submitting a referral does NOT guarantee an appointment.

 

2. Waitlist Process

  • If accepted for review, you’ll be placed on a waitlist.

    • You will be contacted up to 3 times

    • We may leave a voicemail or email

  • If you don’t respond within 12 weeks, your referral is removed, and you must resubmit to request again. .

 

3. Appointment Offer

 

If the referral is accepted, our administration team will contact you by phone. During this call, We will explain the next steps, including scheduling your appointment.

  • Once you accept an appointment:

  • You will receive

    • Appointment letter

    • Information sheet 

    • New Patient Form link

  • Please Check junk/spam folder if you don’t see it

  • These documents will include important details such as the relevant item numbers for billing, fees, what to expect during your first visit, and an overview of the process moving forward.

Note: Completion of the patient form and Consent is required to confirm and proceed with your appointment.

4. Cancellation & Rescheduling

  • Cancel ≥48 hours before → no penalty

  • Cancel <48 hours → Full fee of booked appointment(not covered by Medicare)

  • Reschedule within 48 hour → requires $250 deposit ( only for Initial Appt)

    • Non-refundable

    • Non-transferable

  • Existing patients can book via Halaxy

  • Sibling Referral, please email us to referrals@northstarpaediatrics.com.au

  • General Paediatric health referral, please email us to above email. 

  • Please note same cancellation rules apply
